This address is now TLC veterinary. It is no longer capital Eastside family practice. I just drove by and it's definitely gone.

I've had both Nancy and Maria as providers and I couldn't imagine a better care team. This office does have policies concerning things like being late for appointments or requesting prescription refills so be sure to read your member agreement carefully! Cute little office that is never too busy, with a friendly and helpful staff!
The only good part of this place was my old nurse practitioner who is now gone. I second time I called, I had the front desk lady yell at me on the phone. I think I was trying to schedule a blood draw and asked a question that she didn't know the answer to. Today, when I went to request my files be transferred to my new doctor, they didn't even give me a clipboard. Their counter isn't big enough to write on, and there are no tables.
