That's not the right address
I went to him for an evaluation in August. I called several times about getting results sent to my doctors, he told.me several times he was sending them. He never did. Now he won't answer or return my calls or messages. I even went to his office to see if I could get my records but the door was locked and no one answered even though according to his hours he would have been open.
