I had a appointment here yesterday, it was the worst! My first time and last. When I completed my paperwork usually most doctor or dentist office like to know your past and present medical history. Well I completed the paperwork advising of any present or past issues and was told i needed to complete another one, because I had about 8 checked yes out of probably about 50 questions.
