Well if you are ever made to take short term/ long term disability through your employer and they need documentation dont just take him at his word that he will make sure its handled for you before he even goes home don't worry because he might do like he did me and not fill them out and go on vaccation for 3 week and cost you your job and then not even apologize for it.
