I had my Grandmother at this facility recently and she received phenomenal, personalized care from all departments...Therapy, Nursing, Dietary, etc. The staff were very attentive and communicated with me consistently regarding any changes, any updates, etc. I would not hesitate to put another family member or friend in this facility should they need it. I filed a complaint with the Agency for Health Care Administration regarding the sub par care of my elderly mother at Boulevard Rehabilitation Center in January-February 2014. I found her disheveled, unwashed, and hallucinating when I came to pick her up for a doctor's appointment. I signed her out against medical advice and immediately took her to her primary care doctor who said that the hallucinations were most likely from a urinary tract infection.
