I would give 0 stars if I could. We asked for an appointment because they could get us in the next week. They sent over new patient paperwork by email the next day. I filled everything out, and then on the last page I opened, I saw a release form for my personal medical history information to an "Apostolic Network of Global Awakening." The form was prefilled with someone else's name and information, meaning whoever sent out these packets is not checking to make sure they aren't releasing your personal data.
