This doctor and his entire staff placed lies in my chart to force me out of a place where I was safe from my own internal thoughts and plans. This doctor and his team are why people are afraid and ashamed to seek help for mental health. I was never invited and never knew I could attend treatment team, yet it was stated I refused.
