Original office has moved from Port Charlotte to Punta gorda years ago. And know one seems to know that!! There at Adapt Health building in Punta Gorda 2nd floor .
I submitted an appointment request online several weeks ago as there was no phone number on their website to call for a new patient appointment. I NEVER received a response other than the automated response saying they got my request and someone would get back to me soon.
