Did this provider answer all of your questions? Yes Does this provider have helpful information available online? Yes, but very little Did this provider ever take personal phone calls while in the appointment room with you? He/she would never do that! Their full attention is always on me.

Are you satisfied with your results? Yes! I couldn't be happier! Did this provider have reasonable prices? They were decent, but not very affordable Did the staff make you feel uncomfortable when you called with questions or concerns? Somewhat, they should have been more polite
