Discharged my family member with no improvement and no plan, did not get ven tell us when she was being discharged. Extremely poor planning and communication.
Took an elderly friend to get a psychological evaluation with a provider referral, and the dr. Here didn’t take in consideration his medical and behavioral history. They discharged him knowing he isn’t able to care for himself and displays aggressive behavior sometimes. They knew he didn’t have a place to go, money, or knowledge of the city, they didn’t provide case worker’s information, or help finding him a safe place to stay.
