It’s been over 2 weeks and still no card. My first appointment, I was stood up by the female provider. Made another appointment and she showed up but I have never received my card. I called last Friday and spoke to male provider and he said he would have her call me. She never called back. Just called again and left another message. If she doesn’t return this call and get me my card, I’m reporting them to the state and going elsewhere.

Not even sure why this place is in business, or who it is that even goes there. Call to try and make an appointment and it turns out they are only open on Fridays and you can't get an appointment for weeks in advance. And of course it's the only place in Casa Grande, so now I have to drive all the way into phoenix after waiting 2 days thinking they were closed during the week because of the holidays.

Don't waste your time here. Paperwork was not sent in till many weeks after initial visit. Dr told me half off next time.Tried to get in contact with Dr for over a week and never got a call from dr. Other staff at different office told me they would give me a sweet deal of 100 dollar visit. Went elsewhere.
