First they say you get a two year license but neglect to tell you it still expires after one year and one must go back to the azmmc , pay their fee and the state waives the license fee to them for year two. PLUS, Verizon now says their Cottonwood number is no longer connected. No phone number on the statewide site to call despite large print saying “Call for an appointment “. None working phone number, office was never opened on Wednesday as advertised, waste of time coming from out of town don't bother with this place.

Horrible place, you pay them $400 and they never contact you back. Been waiting over a month when they said it would take a week. Called the AZDHS and they never received the paperwork. Seems to be scammers when you call every day multiple times for a month and never hear back.

Thank you for making this process so easy. Front desk is kind and knowledgeable. The doctor is sweet and intelligent. Great experience...in-and-out in 15-minutes (with appointment). :)
