After reading my visit notes, I am in disbelief. I thought it couldn’t get worse than my actual visit. My visit notes list quite a few inaccuracies. They include, counseling that never occurred, at least one statement I never made, parts of an exam that never happened, and my sexuality/sexual partner gender are listed incorrectly. To see my sexuality listed incorrectly is just awful. Who is just assuming they know who I sleep with? And how can you help me if you don’t even have that correct?
