Not impressed with this location at all. We were first referred to this place back in February 2022 for help by a Social Worker to find placement for my mother who has dementia. The Social Worker showed up and only helped us submit a Medicaid application that we already had filled out. Then stated there was nothing else she could do for us. We were then referred again to this same place for Social Worker help again in July 2022 as my mother has gotten to the point were we are having a hard time taking care of her. I am usually not one to leave a negative review but am SO DISSAPOINTED in the place that I felt I had to. I help care p/t for a lady that has memory issues & has had numerous falls over the last year or so. On her last discharge from rehab we were set up with DOCTORS CHOICE. They were coming 3x a week. Patient had a scheduled follow up with her primary care Dr who had given instructions for labs 2 b drawn a week prior to. Was written 11/5.
