We are in Miami for vacation from another state and had to go for a deep cut on my friends finger. We arrived and the attendant just stared at us no greeting. After we told her the issue she asked for the her insurance card. We then proceeded to fill out paperwork and then wait for about 20mins. She then said after the completed paperwork was in her hand, entered into the computer and insurance card scanned (never asked for ID).
