MY INSURANCE COMPANY KEPT ASKING THEM FOR THE PROPER FORMS BUT THEY REFUSED, DESPITE SEVERAL RETURN-RECEIPT REQUESTED LETTERS THAT I KNOW THEY RECEIVED AND SEVERAL PHONE CALLS, THEY STILL WOULD NOT FILE THE APPROPRIATE PAPERWORK (INS CO SAID THEY HAD TO FILE IT, NOT ME).
