I would never recommend office, under any circumstances. While our initial interactions were ok, concerns became evident down the line, both with the doctor and his office staff. Our daughter is disabled, and while Alpine Dentistry was notified of this right at the start, they continue to expect her to behave "normally" -- displaying complete insensitivity to mental health issues.
