Disappointing that social workers do not relay concerns to nurses or doctors when they say that they would. Informed that a meeting was for doctors, and social workers but yet a family member was there that does not attribute patients care and a discharge date was given, with our concerns not answered. Physical therapists do their jobs, but we felt that the patient should have had more independence when discharged.
