I am giving one star rating, bad review - one star for staff-scheduler, medical assistant, Jamie Velarde PA of Dr. Ali Chahlavi, MD - Neurological Surgery-Jacksonville, FL 32216. Dr. Chahlavi is a fantastic neurosurgeon - he is affiliated with St Vincent's Medical Center - he did six of my bone-spine related surgeries, happy with his performance.
