Just got the link to give a review. Spelled my name wrong. Hoping it is spelled correctly on my chart. Just looked at my prescription. It is spelled wrong there as well. So, it’s wrong on my chart. They have a copy of my DL and my MED INS CARD and it is spelled correctly on both of those forms of ID. I filled out all the necessary paperwork by hand since the iPad was in use when I got there.
