MY husband has UnitedHealthCare PPO insurance which covers annual eye checkup FREE. Yet Dr. Ko's office staff insists on Co-Pay of $55 upfront. Not only is co-pay not required for this in-network check-up visit, yet Ko's office want $55, which is out-of-network copay amount. We called UHC Customer Service, where agent called Ko's office to clarify. Ko's office offered a reduced copay of $35, insisting that this is a 'non routine check-up". Nice fella but he tore my lens capsule, lost the replacement lens into the back of the eye... which is still floating around in the eye. Then put the wrong lens in. Not a happy patient. We all make mistakes, at least Ican still see.
