Asa RN case manager for Community Hospice it is an honor and privilege to provide care to each of my patients. My goals as an RN case manager is not only to provide my patients with excellent care but provide emotional support and comfort to their families. The end of life process can be difficult to manage and everyone needs encouragement and support throughout the process. No patient or family needs to feel alone during their time of need. And with the Community Hospice staff they never will.
