i was referred by my dentist in Maricopa to Acacia for a consultation. Prior to the appointment I gave them my dental insurance information. When I received confirmation of my appointment via email, there was an estimate of the cost included. One of the procedures was not covered per the email. I have received notice from my insurance company that they needed more information to pay a claim. I have called multiple times, to be told that they have sent it to my insurance company. When I have called my insurance company, they have no record of the narrative that they have requested. I call the office again and asked them to just send me the information that my insurance company is requesting and I will send it myself.
