Great place to contact to set up the immigration form I-693 forms (required for I-485). Dr. Muhammad Khan is a civil surgeon and he will be able to provide the sealed envelope for the immigration applications. Note: The immigration portal on the website is delisted but if you give them the call, it is still available. The process is straightforward.
