I’m writing this bcuz Yolanda can’t own her mistake. I went last Tuesday for a standard UA, all she does is collect the sample and send off the documentation stating what panel to use. She only sent the sample, and did not send the paperwork to tell the lab what to test for. So I called Yolanda and she told me that she had Fed-ex and Fax the correct documentation to the lab.
