Due to the following concerns, I made an urgent appointment and traveled over forty minutes: 1. I've been experiencing dizziness lately, and occasionally I feel like a drunk person, which greatly scares me. Mr. Kotturi focused on this problem. 2. During my recent visit to Dr. Kotturi's office, I informed him that I was in the process of establishing a patient history with him and that I would be undergoing surgery on July 5th. I mentioned that I planned to use paid time off for my recovery period, and requested his assistance in clearing me medically and completing my short-term disability paperwork following my paid leave.
