Update Filled the form out from the comment from Verizon below and never received any contact whatsoever. Had extreme issues cancelling service where they left a line connected for over a month that we never had and now we are having to go through litagation with attorneys as they say we owe them over $1000 for service we had less than 30 days (and that's after we paid a nearly $300 bill for 2 lines) and the phones were returned in the boxes. My wife and I decided to try Verizon less than a month ago. In that time we had to call customer support 4 times (on hold 30+ minutes each time) just trying to get my daughters phone set up. We were told 3 separate times over a two week period that a sim card would be sent out the next day and we never received one. They offered our activation fees returned and had to call again when they were never removed from our account and still after that only one activation fee was credited. Picked up a new phone for my wife, she had everything pretty much switched over to the new phone. Went to turn in the trade in and the rep insisted on reswitching everything, now she can't log into her apps and has no way to recover everything. DO NOT let them talk you into transferring your phone contents. Unless you want to be locked out of fb and other apps. Not to mention that they charge you for something you can do yourself for free.
