susan m He booked an assist with a nurse out of my network. Did not advise me written or verbally. Since she was out of network her bill is triple what I had to pay him. Had he chosen an M.D. in my network BC/BS I would have been covered. Desert Surgical owned by Michael Caskey (michelle Caskey was the assistant)...discounted my assist to $300.00....I actually had to pay Weiner $106.00 and anesthesiologist $124.00..Hospital even less.
