There always seems to be surprise billing whenever I go here even for checkups. On my most recent visit they didn't send the bill to my insurance on file (I confirmed with my insurance that it should be covered and they never received it). When I called their billing line and asked for them to re-send it they said that they wouldn't? Not the response I was expecting.
