Wildly unprofessional. I've been a patient for four months. I have seen four different doctors. Whenever I get a new doctor, it is clear that they have not read my chart and have zero clue of what I am seeing them for and what I am seeing them for is a cause for concern! I have NEVER filled out a patient history, despite asking several times if they need this information. Their notes record, "pt history has not been taken", but no effort to take a history.
