I discovered Dr. Casey Hanson entered false and unfounded information into my medical record, information we never discussed and symptoms that I did not exhibit. When I brought this to her attention at my next appointment, she was embarrassed, saying she didn’t realize patients would read their own healthcare summaries. She acknowledged the error, was very apologetic, and agreed to amend my record. Despite that, no amendment was ever made. My request for an amendment was also formally denied by MDIH.
