I was referred to this location by my PCP. Pretty quickly a new patient coordinator contacted me to schedule an appointment. After finding out that I had once been seen by Texas Oncology Southwest in Fort Worth she said that I would have to contact that office and have them complete a transfer of care form and send it to their office.
