First I completed the intake form which asked me my meds, dose and frequency. I put it all on there. Fred did not do his home work, because on of the first things he did was ask me what meds, dose and frequency!!! Then he proceeded to take his sweet time putting in all the information, cutting into MY TIME that I PAY for. I waited 10 min for him to move on.
