I was a patient there from Dec. 2011 until June 26,2012. When i arrived, I was paralyzed from my hips down. My Neurologist told me i would never walk again. I got excellent care while there those 7 months and i WALKED out with the aid of a walker. Now 2015 i am walking unassisted because of the hard work of everyone most especially the Occupational Therapist and the Physical Therapist. They are the best! I now give back by visiting monthly with my certified therapy dog (retired greyhound) and giving encouragement. I would re... read more

DO NOT and I mean DO NOT place your loved ones here. Nursing care is horrible. Patients are left filthy in the halls screaming out to nurses who won't listen and just ignore them. Never seen anything like it in my life. APPALLING!! CNA's are left doing all the work and are kind and caring. While the nurses are just there to administer drugs & sit around and complain about their poor overworked Admins. Physical Therapy on the other hand is excellent even exceptional. HOWEVER nursing is another story entirely.

I wish i could give this place zero stars. DO NOT SEND YOUR LOVE ONES HERE! My grandfather was admitted to Keystone with the knowledge that he was terminally ill with brain cancer. They gave him negligent care, and even yelled at my grandmother in front of a whole bunch of people about "what they were and were not going to do". After my grandfather passed away, my grandmother stopped in to have them fill out one sheet of paper for her nursing home insurance. The administrator very rudely told her that she was not going to wa... read more

The food is lousy... physical treatment personnel are great! Security measures seem adequate. Nurses are nice & caring. Office personnel need an attitude adjustment.

Writes:My mother was sent here for rehab after a fall injured her back. From the er she went to The Palms. She was there one night and suffered abuse at the hands of a CNA who was frustrated at the fact that my mom could not turn to use the bed pan because of her back spasms and hit on my mom's legs leaving a bruise. She turned her so forcefully to her side screaming in pain onto her hip that she broke a few years before. The CNA had so patients with her and told her there was nothing wrong with her and she would prove it. A... read more

My father was admitted for what was intended to be a brief 1-2 week stay, for rehab, after a 4 day hospitalization for an infection. Although walking with a walker when he came in, he was only allowed to walk with rehab and not with staff because he was a fall risk. He was left sitting in an "activity room" for hours on end without any activities or even the tv on and not allowed to be in his room, again because he was a fall risk. One of the aides in this room refused to take him out so he and my mother could visit privatel... read more

This is the worst nursing homes ever. My grandmother was in this facility and the results from her being there was she contacted mursa/mrsa, received a stage 4 ulcer near her rectum, and feet & back sores. No offense to other cultures but, all of their CNA's, LPN's are Haitians and they don't give a dam on how they care for your love ones. Your love ones will definitely be mistreated by them! They will inject something into your love one and can't or won't explain what they have injected. This happen to my grandmother, and t... read more
