My dad went for a hip fracture and ended up here for rehabilitation. Due to the horrible care and neglect, he went from a coherent, able to walk with a cane and able to perform daily tasks to a stuttering, bed bound unable to use the bathroom on his own and petrified of everyone. They do not know how to properly care for anyone. Dad's bloodsugar stayed skyrocketed here. He would tell them he couldn't eat the food they were bringing him and they would get nasty with him. Wood Lake was the facility my family trusted to take care of our mother on two occasions. The staff was caring, kind and very respectful with our mother. To us the staff felt like part of the family. When anyone in our family had any questions or concerns, they were there to take care of our needs right away. I feel that Wood Lake handled themselves very professionally. I am so thankful to Wood Lake Health and Rehabilitation Center for taking such good care of my mom

Smh there is no team work at there and the agency staff acting like they are supervisor when they have a supervisor just no leadership at all and that's sad

Worse 3 days of my grandma's life!!!!! Deceiving. Unprofessional. Abusive. Negligent. Those are words to describe overall experience. Some special people working there though, but they are not the rule, rather the exception. Physical therapy is outstanding.

I would never let my loved ones ever stay here. They do not provide filtered water or proper care for their residents. Medications were not passed out at proper times. Bathrooms were never thoroughly cleaned, where there was feces present on the side of the toilet prior to my loved ones arrival. The bathroom is also where a nurse provided more water for my loved one as well. Some employees you could tell actually cared and provided decent care but I wouldn't take a chance here ever again.

The staff there are unfriendly, a lot of patients in need and no one seems to be around. My mom hates it there. She was at Hawthorne 6 months ago and it was opposite, they were so kind and caring. They have a sign out front looking for help so maybe they are over worked.

I have made several attempts to contact someone at your facility in Greenacres, Florida. I have my friend, Janita Gray being house in your facility and all I'm getting are reports of how she is being mistreated, given the wrong medicationss, and being threatened in general. and the staff particularly your night staff were not the most professional.. I have come up on the Week ends to take her out and around for a few hours and it seemed she was not allowed off your ground. I have since spoken to her case worker, and she can ... read more

To all people looking for a facility to place your dear ones, whatever you do, do not place them in this facility. You will regret it dearly. Of course, that is if you care!!! My mother's primary physician sent her here for 21 days of rehab and I ended up taking her out on the 5th day due to uncleanliness and incompetent staff. The dieticians had no clue on how to properly give my mother her meals and she ended up loosing 9 lbs in her stay. For the past six months my wife has been at Evans. Her memory left ... but there was still a beautiful woman inside. I go visit often. Every time Mary is being well cared for by a professional, loving staff. The place is spotless. It's such a gift to find a place for Mary. God bless them one and all ...
