One Star because Zero was not a Choice!! Negligence in providing timely and essential care is unacceptable and raises serious concerns about the standards of patient care. The Health and Human Services Commission needs to thoroughly investigate this facility. Every patient deserves to receive prompt and adequate attention, especially in moments of distress or urgent need.

Janet Hoffart 08/11/1951 Placed here for skilled nursing 14 days physical therapy. She has many physical issues, diabetes, neuropathy and diagnosed with Anxiety, Stress and Depression. I am her only child and POA FULL TIME caretaker. My mom was here for two days and called me every 10 mins about the neglect and medications not being correct. Tresa the financial manager was a God sent for my Mom and I. She went over and beyond including staying late to help my Mom and I to answer all questions that I didn't understand. What I also loved is if she didn't know the answer to something she went and ask and got back to me immediately. Tresa made me feel like I was a family memeber and not just a client.
