This used to be a very nice clinic now is a complete mess, I give someone name Fred my fisico form, the nurse supposed to call me if they need something else, I when back to pick up the form two weeks later and they lost the paper with my inf, I give it the copy of the form and I didn't know how they fill it out a form from a different agency , 3 weeks later i still waiting for my form and my Job suspended me because I don't know the form done
