Transition of care program is a big joke! My wife spent 8 days in CCU then two in a regular room. Dr told us she needed 4-6 weeks antibiotics so they put in a pick line. After discharge, transition care told us to contact medics to get antibiotics the next morning. Also they setup homehealth. Next morning we attempted to get antibiotics and the prescription was no longer valid because she had been discharged.
