My wife was in a wreck. When I went to file a claim, I find out they had cancelled the insurance on my car replacing it with the car we had just purchased to replace the car that had been totaled 2 months before. The totaled car was still being carried on my policy. I was not notified my coverage was cancelled nor was I given a declarations page or new insurance cards.
