I tied to update my chart information and was sent a blank document with a signature on it. My amendment request isn't in my medical file. I was told I can't ask anymore questions or talk about this issue anymore. They gave me information about my chart and won't clarify. I have a lot of questions. They won't answer. The people in the HIM and Corp Provacy Dept. were rude and sarcastic
