EDIT UPDATE: after 3 months and 9 forms written request for my records. They have not even bothered to reply or even answer an email. They have no regard patient rights or the laws in place to protect them. 3 visits so far and am less impressed with each visit. First they require a power of attorney to be signed upon arrival or they will not see you. Been going to doctors for years and never have been required to sign a POA. After explaining about a severe tailbone injury years prior I am sent for an MRI.
