If you have a family member or anyone you care about do NOT send them to this place! My father was a patient at this facility for approximately 4 days. He was recovering from COVID after being released from the hospital. He had responded well to the treatment and was transferred to Sonterra Health Center to regain his strength. On the fourth day we were notified that he was unresponsive and they couldn't get an O2 sat. on him. He was sent back to the hospital where he was subsequently put on a ventilator and passed away the next day. The doctor in the Emergency Room said he was severely dehydrated and his body temperature had fallen to 94 degrees. Some of the staff we dealt with at Sonterra, including the Director of Nursing were very cold to us and showed no sympathy for our concerns about our Dad. It was almost impossible to get information about him. Our messages were never returned or we were transferred to a phone which nobody answered. In my opinion, this facility does not deserve the rating it has.

The staff was particularly nice and respectful of their patients as well as the family members. Their PT programs seemed to be well done and challenging. I think the staff needs a few more for shift work, but overall this is a trustworthy Rehabilitation Facility that helped our family.

The staff was particularly nice and respectful of their patients as well as the family members. Their PT programs seemed to be well done and challenging. I think the staff needs a few more for shift work, but overall this is a trustworthy Rehabilitation Facility that helped our family.

If anyone’s ideal setting is that of immobile patients sitting or laying in urine and bowel movements until shift change, while family observes staff members sitting at nurses station snacking, chatting and/or on their personal phones is acceptable with you, then this is the place for you.....my mother was rushed to ER twice for untreated UTI and skin breakdown, due to non/attention by the nursing staff.....the UTIs were so far gone, resulting critical renal failure. The second round resulted in seizure....mom had to be transferred to another facility where she is being cared for on a quality level.
Our family’s complaints at Sonterra went unaddressed even after we escalated our concerns to the facility director.

Staff are great and care is great. I worked in care center and this is highly recommended. My mom enjoys it there n rehab great.

Were in Love with the care givers an staff�. The staff from Mrs. Tanya to Nures are wonderful caring to all their patients. We highly recommend if you have a loved one that is in need of 24/7 care. Our mother is so busy with all the daily activities. Has made allot of friends. Thank you

Didn't choose this place -Keep looking – Went to look for a place for mom and checked this new place out. Has been open for at least 2 years and not full, so I did some checking.( I went twice - once during day then at evening time.)Latest state rating only gave them a 3 which is a pretty low score.
The place did not seem home like, seemed a little too "sterile".Dining room looked depressing. Not alot of activities either. Other residents did not seem cheerful . Talked to a couple of staff members (after hours, which is always good and gives a really good picture of how things really are). Seems there has been 3-4 administrators in the building since it opened and alot of staff turnover as well. Not too comfortable with what I found, so chose another place instead. Seems like they are still trying to get their "footing".
Pro: Been open about 2 years/Close to 1604 and other main roadways
