Had gone to this site many times over past couple of years with very good experiences—this past Tuesday not good. Showed up with spouse to get lab drawn and different people at the helm. All of a sudden “we can not accept this requisition-it has no address/fax number”—I could not believe this was happening, paperwork exactly the same as last 4-5 times. I offered to write address/fax number on form but that not good enough. Long story short went to 2nd lab and had blood drawn no problem. Won’t be going back to one in Safeway.
