This provider is not qualified to help with what I needed, and instead of admitting that and offering a referral, he had me fill out forms that appeared to have been photocopied repeatedly that were not relevant to my request before sending a bizarre misdiagnosis to my regular therapist, who considered his write up to be nonsensical/misinformed.
