The most unprofessional physician I have ever dealt with. She takes your chief complaints and words them as she sees fit. I was complaining of throat inflammation and irritation and neck pain. She puts in my medical record sore throat and back pain. My employer requires disability forms to be completed. She refuses to fill out the paperwork with the correct health issues I was complaining of and refuses to give return to work letters and complete FMLA paperwork.
