Two physicians now, Katie and Rose, have both denied me for surgery forms. Rose because she "didn't know me" (what's the point of a medical record then if not to advise you on how to care for patients you're unfamiliar with?) and Katie over parts of the recommended part of the form. When I pointed out how that was recommended and that I still meet the criteria for the required parts, still didn't matter to her and she was "looking out for my best interests."
