The service was done at Utica park clinic at Lewis Ave, Tulsa was an annual physical check up. This annual physical check is covered 100% by my medical insurance. Dr. Saima Salim unethically made a mistake and now she is lying! We have never seen such a disgraceful doctor in our life! Now, if you ask the clinic to check the documents on the day of appointment, you will see with our signature that it’s for annual checkup. Also if you listen the phone recording you will hear that it’s for the annual checkup.
