Does this provider always take that extra step to make you feel special? Yes, I always enjoy my appointments Did this provider promise services he/she couldn't provide? No, they were able to provide everything they said they would Did you feel safe in this provider's care? Absolutely! I knew I was being cared for by an expert! Does this provider promptly return your phone calls? Not really. I usually have to wait a long time Does this provider treat their staff well? Yes, they treat their staff with respect

Never, they go out of their way to address every concern of mine immediately Has this provider ever cancelled your appointment on you last minute? Never, they are very reliable. Did this provider answer all of your questions? Yes Did this provider pressure you to purchase any unnecessary products during your visit? Absolutely not! Their office was nice and quiet. The doctor did not make me feel welcome. He looked sad and serious and didn't smile once. They gave me a sample of a medication to try without telling me of the possible serious and even fatal side effects. The medication was supposed to help me lose weight but the information they gave me along with the medication said people who participated in a study lost 5 pounds in 6 months! Was the waiting area spacious? No, but I can always find a seat Was this provider's office too loud? No, it wasn't too loud Did you leave the office feeling satisfied with your visit? No, I felt confused and uneasy when I left Did this provider ever postpone your appointment? Sometimes, a little too often Are you going to visit this provider again? Only if I couldn't find a better substitute

Did this provider rush your appointment? No Does this provider always put the customer first? Absolutely! They have some of the best customer service I have ever seen Was the waiting area spacious? Yes Was this provider's office too cold? No, it was fine Does this provider treat their staff well? Very! I've never seen an employer treat their staff better

Yes, I was completely at ease Did they follow up with you after your appointment? I had a follow-up appointment, but I scheduled it. Did this provider seem well-trained and experienced? Yes, they seemed very qualified Did this provider seem up-to-date with the current advancements in their field? Yes

Did this provider have reasonable prices? They were decent, but not very affordable Was this provider late to your appointments? Sometimes, it was a little frustrating Did this provider listen to your input and concerns? Yes Were you able to get answers to simple questions from this provider without scheduling an appointment? Yes Did you have to wait long after you arrived for your appointment? I had to wait for a little while.
