My boyfriend receiving cataract surgery Friday. He's been waiting a while to get this done. He has a small 1 page medical history form with a box that needs to be filled out by Dr. Robert C. Wallen. The office said he needs 30 days just to fill a small box out on form. Last time they told me 7-10 days. When he had to get that form filled out by him before the office got him in a day before surgery to fill form out. He should not have to walk around with cataract because he couldn't fill out form.
