Does this provider remember you and your circumstances at every appointment? No. I have to re-explain my circumstances every time Does this provider have helpful information available online? No, just a basic listings page Did this provider ever postpone your appointment? No, they never postpone my appointment Would you refer this provider to a family or friend? No, I wasn't impressed

Does this provider remember you by name? Every single time, even if it's been months since my last appointment!! Would you refer this provider to a family or friend? Yes, I'm going tell everyone about them. I absolutely love this place! Did this provider leave you unattended for an extended period of time? No, I was always attended to Does this provider promptly return your phone calls? Yes

Did this provider listen to your input and concerns? Absolutely! This provider always listens carefully to what I have to say! Was this provider's office too loud? No, it wasn't too loud Did you leave the office feeling satisfied with your visit? Yes Did the staff make you feel uncomfortable when you called with questions or concerns? Yes, they made me feel like I shouldn't have called Was this provider's office clean? Yes everything was neat and sterile
